To request a free session in the clinic, please complete the form below.  You must let us know which times you prefer.  Please choose as many time slots as you are available, and rank them in order of preference (for example, if 9 AM is your first choice, select the number 1 for the 9 AM slot).  If there are several time slots that would work equally well for you, you can mark more than one time slot as your first (or second, or third, etc.) choice.
